Outer Cape Health Services, Inc., not-for-profit community health centers, seeks a Dental Case Manager for a new dental suite to be built at our Provincetown location. On behalf of our registered HIV/AIDS patients, you will coordinate services with their primary care provider(s) as well as with other clinical or social service agencies providing services for them. At least one year experience in a human service-related field and/or with community outeach required. Strong computer skills and the ability to work as part of a team are necessary. |
