RN--Infectious Disease Program Manager--Full Time
Under the supervision of the Lead Nurse, the RN-Infectious Disease Program Manager coordinates patient care among the ID medical team, primary care providers, health center staff and community-based agencies. Provides, timely, clinically-appropriate assessment of patient’s needs, develops individualized care plans for provider’s review, and determines best methods of implementing the plan – e.g. within OCHS or by an outside provider or agency. Maintains effective, collaborative relationships with patients, providers, fellow nurses, staff, families and outside agency personnel. |
